Dear Parents and caregivers,

Welcome to the class placement request form. Please note, a separate form will need to be completed for each child you wish to submit a request for. Please consider your requests carefully and only submit one form for each child. 

Requests will be considered and where possible acted upon. However; with only a finite number of class options and creating classes that are balanced academically, socially and behaviourally the good of the whole group will take priority over the individual. Therefore parent requests are not guaranteed.

When setting classes we consider student identified friendships, teacher input and parent requests.  

Please follow the instructions of each section carefully and complete the sections that are applicable to you. Requests forms will not be accepted after 9th November.
