If you want to play in a sport in the Eden Hills Primary School After School Sports Program, please have a PARENT or CAREGIVER complete this form.

It is really important to enroll as soon as possible because we have to nominate teams with each sporting body. 

If this form is not completed your child might miss out on a place or we might be too late to nominate a team.

You only need to fill it out once, you do not need re-register each term unless you have to update your contact or medical information.
